Care Management
Supporting Healthier Pregnancies in Pender County
Care Management for High-Risk Pregnancies (CMHRP) is a statewide program in North Carolina designed to support healthy pregnancies, safe deliveries, and strong starts for babies.
Our CMHRP care managers—registered nurses or social workers—partner with you and your prenatal provider to ensure you receive the care and resources you need during pregnancy and after delivery.
Who is Eligible?
You may qualify if you:
- Have Medicaid or a NC Managed Care Health Plan (Healthy Blue, Carolina Complete Health, UnitedHealthcare, AmeriHealth Caritas, or WellCare)
- Are Medicaid-eligible and meet screening criteria such as:
- Chronic medical or behavioral health conditions
- Fetal complications
- History of low birth weight delivery (under 5.5 lbs)
- History of preterm delivery (before 37 weeks)
- Late entry into prenatal care (after 14 weeks)
- Multiple pregnancy (twins, triplets, etc.)
- Substance use or tobacco use
- Unsafe or unstable living environment (domestic violence, abuse)
How a CMHRP Care Manager Can Help
- Coordinate care and connect you to services like medical care, food, transportation, or housing
- Support you in managing stress and life challenges
- Offer guidance on healthy pregnancy and postpartum care
- Provide trusted information on family health, infant care, and development

How Can CMARC Benefit Your Family?
CMARC is a program offered at no charge for children birth to 5 years of age who:
- Have special health care needs; and/or
- Experience the Infant Neonatal intensive Care Unit (NICU); and/or
- Are dealing with adverse childhood events; and/or
- Are referred by the child’s doctor and/or health plan
When a referral is received, the care manager will talk with you to determine whether your child and family could benefit from CMARC services. For eligible families who want CMARC services, a CMARC plan of care is developed by the family and the CMARC care manager.
What Care Managers Do
CMARC care managers are here to support you because you are the expert on your child. Our trained nurses and social workers take the time to listen, understand your family’s unique needs, and help you get connected to the right care and resources.
Your care manager will:
- Recognize you as your child’s first and most important advocate
- Learn about your child’s needs, concerns, and goals
- Help you build a strong relationship with your child’s doctor (medical home)
- Coordinate with health care providers to make sure your child receives appropriate services
- Offer referrals to community resources like WIC, behavioral health, childcare, transportation, and more
- Support families facing stressful situations such as housing, finances, or caregiving challenges
- Provide information on child development, health topics, and parenting
- Take time to talk with you about your child’s growth, behavior, and milestones
- Answer your questions and help you make informed decisions for your child’s well-being
CMARC works with all types of families—including parents, grandparents, foster families, and other guardians.

- You have concerns about your child’s behavior.
- You have questions about your child’s special health care/medical needs.
- You need information on community resources that could benefit your child.
- You need help connecting with local providers.

- home visits
- phone calls
- visits to the doctor with you
- other types of contact
