Care Management for At-Risk Children (CMARC)

How Can CMARC Benefit Your Family?

CMARC is a program offered at no charge for children birth to 5 years of age who: 

  • Have special health care needs; and/or
  • Experience the Infant Neonatal intensive Care Unit (NICU); and/or 
  • Are dealing with adverse childhood events; and/or 
  • Are referred by the child’s doctor and/or health plan

When a referral is received, the care manager will talk with you to determine whether your child and family could benefit from CMARC services. For eligible families who want CMARC services, a CMARC plan of care is developed by the family and the CMARC care manager.


What Care Managers Do

CMARC care managers are here to support you because you are the expert on your child. Our trained nurses and social workers take the time to listen, understand your family’s unique needs, and help you get connected to the right care and resources.

Your care manager will:

  • Recognize you as your child’s first and most important advocate
  • Learn about your child’s needs, concerns, and goals
  • Help you build a strong relationship with your child’s doctor (medical home)
  • Coordinate with health care providers to make sure your child receives appropriate services
  • Offer referrals to community resources like WIC, behavioral health, childcare, transportation, and more
  • Support families facing stressful situations such as housing, finances, or caregiving challenges
  • Provide information on child development, health topics, and parenting
  • Take time to talk with you about your child’s growth, behavior, and milestones
  • Answer your questions and help you make informed decisions for your child’s well-being

CMARC works with all types of families—including parents, grandparents, foster families, and other guardians.
